Adult Membership Includes: 
  • 1 Comprehensive Exam
  • 2 Cleanings (In the absence of periodontal disease)
  • 1 Panoramic Radiograph
  • 2-4 Bitewing Radiographs (as needed)
  • 2 Periapical Radiographs (of front teeth)
  • 1 Emergency exam (During normal operating hours)
  • 20% off all Additional Treatments

Periodontal Health Membership

$399/year

Periodontal Health Membership Includes:
  • 1 Comprehensive Exam
  • 20% off the initial deep cleaning (Scaling and Root Planing)
  • 3 periodontal maintenance cleanings per year
    ‍*frequency of cleanings based on dentist diagnosis and prescription
  • In office periodontal adjunctive treatments prescribed by the dentist. Examples include any mouthrinses, locally applied antibiotics, ect.
  • 1 Panoramic Radiograph
  • 2-4 Bitewing Radiographs (as needed)
  • 2 Periapical Radiographs (of front teeth)
  • 1 Emergency exam (During normal operating hours)
  • 20% off all Additional Treatments
